While this questionnaire was previously used for E-DUBBLE's fan club membership application, it can now be used for a more appropriate purpose given the circumstances. If you would like to fill it out, the form data submitted will be sent from this website directly to Tommy's inbox, and since all boxes are merely optional (including name and e-mail), you can remain completely anonymous if you would like to. Tommy will compile a selection of the submissions to possibly be utilized for one or both of (a) a memorial tab that we can eventually add to this website, and/or (b) perhaps make into a compilation book that could eventually be presented to the immediate family of Evan. So if you want to say anything meaningful to or about Evan, feel free to RESPECTFULLY utilize this outlet. NOTE: There is a box you can check if you would prefer your submission not be considered for either one or both of options (a) or (b) above.
